West Jordan’s Passport Palooza

On Saturday, May 20th, West Jordan City will be offering walk-in passport appointments from 9:00 AM to 12:30 PM. Our office will start processing applications at 9:00 AM and will take the last walk-in at 12:30 PM. There will be a sign-in list available to help our office keep track of who is next in line. Please understand that, because this is a walk-in event, you may have to wait a significant amount of time at the office. If you are not prepared or do not have all of the documents ready, we WILL NOT be able to process your application at that time. If you are not on the sign-in list by 12:30 PM, we WILL NOT be able to process your application. For information about passports and to fill out the applications, please visit travel.state.gov
**If you fill out the application online with the form filler, you MUST print out the application and bring it in person. Using the online form filler does not submit your application.

What do I need to bring?
If you are using Form DS-11:
- The Application Form filled out in black ink with no mistakes, cross outs, or whiteout

- Bring a Photocopy of U.S. Citizenship Evidence (we can make copies in our office for you)
- Present Physical ID

- Check per application to pay for the application fees made out to the U.S. Department of State
Applying with your child who is under the age of 16 (DS-11)
- The Application Form filled out in black ink with no mistakes, cross outs, or whiteout
- Original or certified copy of proof of U.S. Citizenship for the applicant
- Photocopy of U.S. Citizenship Evidence (we can make copies in our office for you)
- Show parental relationship
- You must submit original or certified documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport, such as:
- U.S. Birth Certificate (also evidence of U.S. citizenship)
- Foreign birth certificate
- Adoption decree
- Divorce/Custody decree
- You must submit original or certified documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport, such as:
- Present Physical ID
- Photocopy of ID (we can make copies in our office for you)
- Show Parental Consent
- BOTH parents/guardians must authorize the issuance of the child’s passport. The best way to do this is for both parents/guardians to go with the child in person to apply for the passport.
- If one parent is unable to appear, they can give permission by completing Form DS-3053. You must submit the completed form with the child’s passport application as well as a photocopy of the front and back side of the ID that they presented to the notary.
- Photo (we can take photographs in our office for $10+ tax)
- Check per application to pay for the application fees made out to the U.S. Department of State
Applicants Age 16 & 17 (DS-11)
- The Application Form filled out in black ink with no mistakes, cross outs, or whiteout
- Original or certified copy of proof of U.S. Citizenship
- Photocopy of U.S. Citizenship Evidence (we can make copies in our office for you)
- Show Parental Awareness
- If you have your own current identification document (ID), you may apply for a passport. You must show at least one legal parent or legal guardian is aware that you are applying for a passport.
- How to show parental awareness:
- A legal parent or legal guardian appearing with you in person when you apply for your passport. The agent or employee accepting the application will ask your legal parent or legal guardian to sign Form DS-11. The parent or guardian will need to bring their physical ID.
- Submit a signed note from your legal parent or legal guardian with a photocopy of their ID
- Submit proof that your legal parent or legal guardian is paying your application fees (Example: parent or guardian name is written on the check or money order)
**You must bring your Birth Certificate or Certified documentation that lists the parent or legal guardian
- Present Physical ID
- Photocopy of ID (we can make copies in our office for you)
- Photo (we can take photographs in our office for $10+ tax)

What form should I use?
- Form DS-11
- Applying for your first U.S. Passport
- Applying with your child who is under the age of 16
- Your previous U.S. Passport was issued when you were under the age of 16
- Your previous U.S. Passport was lost, stolen, or damaged (if so, please also fill out form
- Your previous U.S. Passport was issued more than 15 years ago
- Form DS-82
- You can use this form if you respond “Yes” to all statements below about your most recent passport:
- I can submit it with my application
- It is not damaged (other than normal wear and tear)
- I have never reported it lost or stolen
- It was issued within the last 15 years
- It was issued when I was age 16 or older
- It was issued in my current name, or I can provide another document such as a marriage certificate or divorce decree that is evidence of my name change
- It was not limited to less than the normal 10 year period of validity for an adult passport due to multiple passport thefts or losses, or due to issues with passport damage or mutilation
- You can use this form if you respond “Yes” to all statements below about your most recent passport:

What kind of documents are proof of U.S. Citizenship?
- Full validity, undamaged U.S. Passport (expired passports are accepted). Full validity means the document is/was valid for 10 years for adults and 5 years for children under 16.
- U.S. Birth Certificate that meets the following requirements:
- Issued by the City, County, or State of Birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)’ full names
- Has the date filed with registrar’s office (must be within one year of birth)
- Has the registrar’s signature
- Has the seal of the issuing authority
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship
**Birth Certificate abstracts are NOT accepted, and you cannot submit digital evidence

What forms of ID are accepted?
- Valid or expired, undamaged U.S. passport book or passport card
- In-state, fully valid driver’s license or enhanced driver’s license with photo
- Certificate of Naturalization
- Certificate of Citizenship
- Government employee ID (city, county, state, or federal)
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Matricula Consular (Mexican Consular ID) – commonly used by a parent of a U.S. citizen child applicant
- U.S. Permanent Resident Card (Green Card) – commonly used by a parent of a U.S. citizen child applicant
- Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
- Enhanced Tribal Cards and Native American tribal photo IDs
- Note: you may be asked to present an additional ID when presenting one of these three documents: 1) In-state, fully-valid learner’s permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver’s license with photo.
Please note:
- Some states now issue digital ID documents (also known as mobile driver’s licenses or mobile IDs). We cannot accept these digital IDs when you apply for your U.S. passport. You must continue to submit your physical, photo ID and a photocopy of the ID.
- If you present an out-of-state ID, you must present an additional ID.

It says that you need to bring a check for the application fees. Can I pay with a card?
No, the U.S. Department of State requires a personal check, cashier’s check, or a money order made out to the U.S. Department of State to pay for the application fee.

Child Applicants (under the age of 16):

Note: the 1-2 Day Return Delivery is only applicable to the Passport Book

Are there any additional fees associated with applying?
Yes, there are additional fees you will need to pay to the City of West Jordan. These fees can be paid with a check, cash, or card.
If you are using Form DS-11, you will need to pay a $35 Execution fee per application.
If you are using Form DS-82, you will only need to pay for the shipping of the application. The price varies according to USPS Priority Shipping
If you need a photo taken, we can take it for $10+tax
